Over Coming Challenges in IV Thrombolysis for Ischemic Stroke. A Single Centre Study

Stroke is the 2nd leading causing of death and 3rd leading cause of disability globally,effecting over 12 million people anually with ischemic strokes accounting for approximately 85 % of all cases. IV thrombolysis with in the window period and in the extended window period according to recent guidelines reduces the morbidity and mortality.Time is brain is the concept with which we have done this study. Yet it remains underutilised in many regions due to lack of awareness,difficulty in thrombolysing challanging cases and lack of transport facilities and financial issues. Here we present the ways and means by which we could overcome some of the challanging issues and increase the percentage of thrombolysis. The statistics show less then 5% thrombolysis in developing countries which have gone up to 15-20% in some of the developed countries.In our centre thrombolysis rates have been improved from 9% in the year 23-24 to 20% in the year 24-25.we have followed a series of targeted interventions including public awareness campaigns,educational vedios by neurologists and testimonials from successfully treated patient.we implemented thrombolysis during both standard and extended window period using tenecteplase at a dose of 0.25 mg / kg/bwt.several challanging cases  including patients on anticoagulants for a single and double valve replacements with even inr 2.5 were successfully managed with induvidualised risk assessement. Our experience demonstrates public engagements,procol -driven evaluation and case specific decision making enhanced the access to thrombolytic therapy and improve stroke out comes even in high risk populations. We can improve further in the percentage of thrombolysis,if we can provide advanced stroke ambulance facility and trained stroke team,telemedicine and using arteficial intillegence. We discussed in detail about the various challanges in thrombolysis and how we could overcome and atained successful thrombolysis in this article.
